Registration

Our staff is here to help you, your care team and health insurance provider gather all the information necessary to move forward with surgery and manage your health. When you register, our front-office staff will update your information in our records and help you file accurate informational forms for your insurance company. Keeping your medical records up-to-date also allows your doctor to contact you more easily, if necessary. Please come about 15 to 30 minutes before your scheduled outpatient appointment. If you need to arrive at a different time for specific departments, we will be sure to let you know.

Tips to help registration go smoothly:

  • Make sure your photo ID, insurance card, referrals, co-payments, and any other related paperwork are handy.
  • Tell our staff the name of your primary care doctor and the name of your surgeon.
  • Notify our staff if you have any drug or latex allergies.
  • Inform our staff of any tests or test days scheduled by your surgeon.
